The project was funded under Horizon 2020 from the European Commission (EC). Thirteen partners from eight European countries worked together for four years to achieve the two main SUSTAIN\u2019s objectives:<\/p>\n<ul>\n<li>To support and monitor improvements to established integrated care initiatives for older people living at home with multiple health and social care needs, and in so doing move towards more person-centred, prevention-oriented, safe, efficient, and coordinated care;<\/li>\n<li>to contribute to the adoption and application of these improvements to other health and social care systems, and regions in Europe.<\/li>\n<\/ul>\n<p><strong>The SUSTAIN consortium is glad to announce the SUSTAIN Final Conference<\/strong>, which will take place on Wednesday, 13th March 2019 at the European Economic and Social Committee (Room JDE62). You are warmly invited to take part in this one-day event in Brussels, Belgium.<\/p>\n<p>During the conference, the SUSTAIN project will be presented in all its aspects: the research carried out within its framework will be showcased; the integrated care sites which took part in the project will be introduced; and the achieved results and project outcomes aimed at delivering better integrated care for older people, including the SUSTAIN Roadmap for Integrated Care.<\/p>\n<p>Further information on the conference programme will be released closer to the date; therefore, you are kindly invited to save the date, register and check the\u00a0<a href=\"http:\/\/www.sustain-eu.org\/\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">SUSTAIN website<\/a>\u00a0and our Twitter account\u00a0<a href=\"https:\/\/twitter.com\/SUSTAINeu\" target=\"_blank\" rel=\"nofollow noopener noreferrer\">@SUSTAINeu<\/a>.
